If I measure the "inner" shell it's 12", but if I measure the "outer" shell or the hoop it's more like 13"

Well I dont know then, id imagine you'd measure from bearing edge to bearing edge.

but it seems like it should be exactly 13" not almost 13".

Drum shells are sometimes slightly under-sized. It allows the flat part of the drumhead to rest against the bearing edge instead of the curved part of the head. So if your shell measures 12 3/4" or so, it's a 13. The hoop should be 13" from inner edge to inner edge.
